2. One (1) recommendation letter from your former professors/supervisor. OU Form 2 Recommendation Form |
3. Permit to study from head of your agency if employed. OU Form 3 Permit to Study |
4. Official Transcript of Records and Transcript of Records (OTR/TOR) |
5. Payment of Application Fee of ₱100 for Local and ₱1,500 for foreign nationals (Non-refundable). |
6. An essay of about 300 words indicating your purpose for applying to the distance education program and your plan after completing the degree. |
7. Latest identical ID picture (2” x 2”) with plain white background. |
Additional Requirement for Qualified Applicants |

1. Original Certificate of Transfer Credential/ Honorable Dismissal from the last school attended (not required for BSU graduates). |
2. Two (2) photocopy of PSA birth certificate and marriage certificate (for married) |
3. Valid visa for foreign nationals. (Foreign students only) |
