Applicants must submit or accomplish the following:

| 4. Permit to Study (OU FORM 3) – from the head of your agency (if employed) |
| 5. Certified True Copy of Official Transcript of Records/Transcript of Records (OTR/TOR) |
| 6. Application Fee of ₱100 for Local and $25 for foreign nationals (non-refundable) |
| 7. An essay indicating your purpose for applying to the distance education program and your plans after completing the degree (at least 300 words with your name and signature at the bottom) |
| 8. Clear photocopies of PSA-issued Birth Certificate (preferably a QR-Coded PSA Birth Certificate if the content not clear and readable) or Local Civil Registry. |
| 9. Clear photocopies of Marriage Certificate (for married women). |

| 1. Original Certificate of Transfer Credential/Honorable Dismissal from the last school attended (not required to BSU Graduates) |
| 2. Applicants may submit a Medical Certificate issued by any government physician. |
| Additional Requirements for INTERNATIONAL APPLICANTS |
| 1. Birth Certificate or its equivalent and Marriage Certificate (for married women) duly authenticated by the Philippine Foreign Service Post. |
| 2. Photocopy of passport bearing the bio-page, the latest arrival with a valid authorized stay with at least one (1) month. |
| 3. Result of TOEFL/IELTS where English is not the medium of instruction (if applicable). |
| 4. Five (5) copies of hand-written Personal History Statement (PHS) duly signed by the applicant, both in English and in his national alphabet accompanied by his personal seal, if any, and containing, among others, his/her left and right thumbprints and a 2×2 inch colored photo on a plain white background taken not more than six (6) months prior to submission. |
| 5. Notarized Affidavit of Support including bank statements or notarized notice of grant for institutional scholars to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses (if applicable). |
| 6. Personal letter of intent (in hand-writing) addressed to the University President (through the BSU – Open University). |
| 7. ORIGINAL copy of Red Ribbon (for transferees, submit a photocopy) which includes: 1) Official Transcript of Records (translated in English), with Grading System or the equivalent of grades duly authenticated by the Philippine Foreign Service Post located in the student applicant’s country of origin or legal residence (must include valid contact numbers and email address from the former school/university for verification purposes); 2) Diploma or Certificate of Graduation; and 3) Certificate of Good Moral Character or equivalent. |
| 8. Applicants may submit a Medical Certificate issued by any government physician. |
