Applicants must submit or accomplish the following:<\/p>\n\n\n\n

For Initial Evaluation (2 copies each)<\/strong><\/td><\/tr>
1. 2×2 ID Picture<\/strong> with name and signature.<\/td><\/tr>
2. Admission Form (OU FORM 1)<\/a><\/strong><\/td><\/tr>
3. Recommendation Form (OU FORM 2)<\/a><\/strong> –<\/strong><\/em> from former professors\/supervisor<\/em><\/td><\/tr>
4. Permit to Study (OU FORM 3)<\/a><\/strong> –<\/strong> from the head of your agency (if employed)<\/em><\/td><\/tr>
5. Certified True Copy<\/strong> of Official Transcript of Records\/Transcript of Records (OTR\/TOR)<\/em><\/td><\/tr>
6. Application Fee of \u20b1100 for Local and $25 for foreign nationals (non-refundable)<\/em><\/td><\/tr>
7. An essay <\/strong>indicating your purpose for applying to BSU Open University<\/strong> (at least 300 words with your name and signature at the bottom)<\/em><\/td><\/tr>
8. Clear photocopies of PSA Birth Certificate\/LCR and Marriage Certificate<\/strong> (if applicable)<\/em><\/td><\/tr><\/tbody><\/table><\/figure>\n<\/div><\/div>\n\n\n\n
Additional Requirements for Qualified Applicants<\/strong><\/td><\/tr>
1. Original Certificate of Transfer Credential\/Honorable Dismissal<\/strong> from the last school attended (not required to BSU Graduates)<\/em><\/td><\/tr>
2. Valid VISA<\/strong> for foreign nationals (Foreign students)<\/em><\/td><\/tr><\/tbody><\/table><\/figure>\n<\/div>\n<\/div>\n\n\n
